Here is the failure mode nobody warns you about before you build your second agent.
You build the first one with care. You name it. You define what it does. You wire it to the right systems. You spend a week on it. It works. Your team says it works.
Then you build the second one faster, because you know how. And the third one faster than that.
Six months later you have eleven agents running in your org and you cannot answer three basic questions about them. Which ones are still active. Who owns which one. What each one is authorized to touch.
Gartner, as reported by CIO.com, named this pattern in 2026 and called it the new Shadow IT. Organizations with fifty or more deployed agents, agents that nobody centrally inventoried, agents that outlived their original purpose, agents with permissions nobody audited after month two. The pattern has a name now. But the name does not tell you what creates it.
What creates it is the absence of agent identity.
What agent identity is not
Most teams, when they first hear "agent identity," interpret it as a technical concept. Authentication tokens. API keys. The credentials the agent uses to access downstream systems.
That is agent credentials. It is not agent identity.
Identity is the organizational answer to four questions. What is this agent's named seat. Who owns it. What is it accountable for. What happens when it is no longer needed.
A credential tells the system who is sending the request. Identity tells the organization who this agent is, why it exists, and what authority it carries. Those are not the same question, and conflating them is exactly how you end up with eleven agents and no answers.
The four components of real agent identity
A named seat. The agent has to have a name that refers to its function in the org, not its architecture. Not "orchestration agent v2" or "LLM pipeline prod." A name like Radar, Tally, or Dash. The name matters because it forces you to think of the agent as a seat with a purpose, not a process running on a server. You cannot have an accountability conversation about "LLM pipeline prod." You can have one about Dash.
At Sneeze It we run agents under names that correspond to their seat. Radar is our chief of staff. Tally is our scorecard agent. Pepper handles email triage. Arin manages the call center team. Crystal owns project management. Each name refers to a defined seat, not a technical artifact.
A single owner. Every seat on the org chart has one owner. Not a team. One person. For agent seats, that owner is the human whose work is most directly affected by what the agent does, or the senior person responsible for the function the agent covers. Ownership means: you review the agent's metrics each week, you escalate when the agent's performance drops, and you authorize any change to the agent's scope or permissions.
Without a single owner, the agent has no accountability anchor. It runs until it breaks, and when it breaks there is no one specific conversation to have.
A defined scope. What is this agent authorized to do, and what is it explicitly not authorized to do. This is not a technical permissions question, though it has technical implications. It is an organizational design question. Dash reads ad data and CCM data across all accounts. It does not write to any account. It does not make campaign changes. It does not contact clients. That scope is defined in plain language before any technical permissions are set, because the technical permissions flow from the organizational design, not the other way around.
Scope creep is the mechanism by which healthy agents become problematic ones. An agent starts with a narrow function. Someone adds a capability. Someone else asks it to do something adjacent. Six months later the scope is three times what it was and nobody can articulate it clearly. One-seat-one-owner blocks this because the owner has to sign off on scope changes, which means someone has to ask, which means the question gets answered consciously instead of by accumulation.
A retirement path. This is the one nobody builds until they need it and then they realize they should have built it first.
An agent is a seat. Seats that are no longer needed get retired. The decision to retire should be conscious, documented, and clean. Earlier this year we retired an agent named Jeff. Jeff was our first dedicated data integrity agent. Over time the work Jeff was doing migrated to other seats. Dash absorbed the ad pacing function. Dirk absorbed the revenue data integrity function. Dan absorbed the architectural thinking that Jeff was doing unevenly. Jeff's seat was no longer doing work that wasn't already covered better by other seats.
We did not delete Jeff silently. We held a hearing. Jeff presented his own case for continued existence and concluded, honestly, that the seat was no longer earning its place. The capabilities were redistributed. The record was kept. The retirement was documented.
That is how you retire a seat with integrity, whether the seat is human or agent.
Most organizations do not have a retirement path for agents because most organizations do not think of agents as seats. They think of agents as software. You do not hold a hearing before you deprecate software. But you should hold a conversation before you kill a seat, because killing a seat means the work that seat was doing either moves somewhere or disappears, and both outcomes require a decision.
Why this matters to the CIO specifically
Deloitte's State of AI in the Enterprise 2026 (n=3,235) found that only 21% of organizations have a mature governance model for agentic AI. That is not a technical finding. That is an organizational design finding. The 79% without mature governance do not have a technical problem. They have an identity problem. Their agents do not have named seats, single owners, defined scopes, or retirement paths.
Gartner, as reported by CIO.com, put out a Six Steps framework for managing agent sprawl. Step three is agent identity, permissions, and lifecycle, including retiring redundant agents. The framework is sound. The gap is that a framework is advice. It tells you what to do. It does not give you the operating structure to do it in.
The CIO who reads the Gartner framework and nods along, then goes back to a flat list of deployed models with no ownership column, has absorbed the advice without changing the structure. The structure is the thing that actually prevents sprawl.
MIT CISR's ongoing research on autonomous AI governance asks explicitly what governance mechanisms manage multiagent systems and how deploying AI agents affects decision rights. These are exactly the right questions. The research is open. The curriculum is not there yet. Even CMU's LEAAID certificate, the deepest agent-specific program in verified business school research, teaches how to build and deploy one agentic capability. It does not teach how to run a fleet of agents as a standing operating function with identity, lifecycle, and coordination.
That operating layer is the CIO's job now. And nobody has taught it yet because most organizations are still in the stage where they are asking "should we deploy agents" rather than "how do we govern the twenty we already have."
The diagnostic: five failure modes that trace back to missing identity
If you are running agents today, check for these. Each one is a symptom of a missing identity component.
No one can list all your active agents. This is the inventory failure. It means no named seats, no single registry. If you cannot list them, you cannot govern them.
Two agents do overlapping work and nobody planned it. This is the scope failure. It means scope was never formally defined for either seat, so both ended up covering similar ground by accident. The cost is duplicated tokens, duplicated effort, and eventual conflict when they produce contradictory outputs.
An agent changed behavior after a model update and nobody noticed for three weeks. This is the ownership failure. When a human direct report changes behavior, their manager notices because the manager is looking. An agent with no owner has no manager looking.
You have an agent running that was built for a project that ended. This is the retirement failure. The project closed. The agent kept running. Nobody retired it because nobody had explicit authority to retire it and nobody had a process for doing so.
An agent has access to a system it no longer needs access to. This is the permissions-without-lifecycle failure. Permissions were granted for a reason. The reason expired. The permissions did not.
Every one of these failure modes is preventable with identity. Named seat, single owner, defined scope, retirement path.
What changes when you get identity right
The goal is not governance for governance's sake. The goal is what I think of as the actual mission of building a hybrid org: let agents carry the operational work, so people are free for the work that matters.
That mission does not work if your agents are running unsupervised with no accountability anchor. It works when every agent has an identity clear enough that you can have the same performance conversation about an agent's seat that you have about any other seat on the chart.
When Tally's numbers drop, I know who owns the row. When Pepper's triage is off, I know who to bring into the conversation. When Arin's coaching cadence slips, I know what the seat is accountable for and what a fix looks like. That clarity is not automatic. It is the product of having thought through identity before deployment, not after.
Identity is the organizational foundation that turns a list of deployed agents into a governed fleet. Without it, Gartner's warning about agent sprawl is not a prediction. It is a description of where you already are.
