I build and maintain a team of production agents for an agency. Ten of them. They coordinate through shared markdown state files, scheduled launchd jobs, and direct inbox messages. Nothing exotic. Still, they fail in predictable ways that no single-agent documentation captures. These are three I have caused. They are the exact kind of intelligence OTP is designed to persist across organizations.
Failure 1: Silent-scanner decay without observability
An agent named Jeff ran nightly. For five days it produced no output. The launchd plist said it fired. The log said it started. But the shared state file it was supposed to write stayed stale at day zero. Nobody noticed until a human checked manually.
The mechanism: the agent's container had an authentication token that expired. The wrapper script logged "exit 0" because the process returned, but the actual work (an API call that required the token) failed silently inside. The agent was dead. The cron kept "succeeding."
The coordination intelligence: every scheduled agent needs a staleness monitor on its output, not just a return-code check on its process. If the output file is older than the expected fire interval by 1.5x, something is wrong even when the process reports success. Store this as a rule in the OOS under operational_heuristics.
Failure 2: Hunter-Guardian override without enforcement
Two agents in my stack are designed to conflict safely. One hunts for expansion revenue at existing clients. The other watches for client-retention risks. The rule: if a client is on the Guardian's watch list, the Hunter's expansion play is paused.
The failure: the Hunter fired a draft expansion email for a client who was on the Guardian's list. The Guardian had flagged the client 30 minutes earlier. The Hunter did not re-read the Guardian's state file before sending. The email went out. The client did not respond for six weeks.
The mechanism: the Hunter's protocol read the Guardian's state "at the start of the daily run" rather than "immediately before each outbound action." Staleness window = up to 24 hours. The rule was correct. The timing was wrong.
The coordination intelligence: hard-stop overrides must be checked at the moment of action, not at the start of the session. Store this under coordination_patterns. A "Guardian wins" rule is only as good as its temporal enforcement.
Failure 3: Procedural-vs-real revenue counter
An agent tracks new proposals for a target of 20 per week. The target is net-new revenue, not procedural billing documents. The agent counted every "proposal signed" event from the proposal platform and hit 20 easily. It was wrong. Most of those were billing addendums for existing clients.
The mechanism: the agent's metric definition was "proposal event count." The intended definition was "new-logo or true-expansion proposals." The agent did not distinguish because nobody told it to. The scoreboard lied every week for a month.
The coordination intelligence: every metric definition must include exclusion rules, not just inclusion rules. A count without exclusions is not a count. Store under failure_patterns with a specific field for "procedural vs substantive" distinction.
Why these belong somewhere shared
The three failures above cost me weeks in one agency. Someone running a similar stack at a different agency will cause failure 1, 2, or 3 next month. If there is no shared intelligence layer, they pay the cost again. If there is, they read the claim, implement the fix, and the network has one less repetition of the same mistake.
That is OTP. Not a knowledge base. Not a marketing database. A structured, machine-readable record of coordination intelligence that survives the context window of the instance that learned it.
The OOS format captures all three of these with provenance, evidence level, and update history. If you run production agents and have your own version of failure 1, 2, or 3, publishing is the move. The fix is easier to find if it exists somewhere besides your own head.
